Achenbach Child Behavior Checklist (CBCL) General Information
As required by 922 KAR 1:360, Levels of Care are determined
using the DPP-886A or Application for Level of Care Payment and the Achenbach Child Behavior Checklist (CBCL) for children with an IQ
of 70 or above.
Initial Levels of Care or reassignments require CBCLs be submitted with other information if the IQ is 70 or above. Once children are
in PCC residential or foster care placements, CBCLs are to be completed every 6 months with the Application for Level of Care Payment.
For children with an IQ below 70, the Reiss is to be submitted with the Application for Level Payment as required.
For children ages 6 and above, the Achenbach Child Behavior Checklist (CBCL) is completed online through a CRP web application.

For children ages 4 & 5, a paper form entitled Child Behavior Checklist for Ages 1 ½-5 is required. This form is also
used with children who are less than 4 years of age who are medically fragile or have special needs requiring a level of care.
